Steven Chudik, MD
Illinois Bone and Joint Institute
Board certified in orthopaedic surgery and sports medicine, Dr. Steven C. Chudik is renowned for his arthroscopic surgical expertise and innovative techniques for shoulder, knee and sports-related injuries. In practice since 2002, Dr. Chudik is a partner at Hinsdale Orthopaedics, a division of Illinois Bone & Joint Institute, and treats patients from offices in Westmont and Western Springs, Illinois. Dr. Chudik works with each patient to develop the best care and rehabilitation plan for his/her life and functional goals.
Dr. Chudik earned his medical degree from the University of Chicago Pritzker School of Medicine, he completed his residency in orthopaedics at the University of North Carolina Hospitals, and a fellowship in shoulder surgery and sports medicine at the Hospital for Special Surgery, New York, an affiliate of Cornell University.
A researcher and innovator, Dr. Chudik is dedicated to improving treatments and outcomes for his patients. He also is a clinical assistant professor and mentor for medical students in an honors research program at Loyola University Chicago Stritch School of Medicine, a research associate at Argonne National Laboratory and Edward J. Hines VA Musculoskeletal Biomechanics Laboratory, and a faculty instructor for the University of Illinois at Chicago Sports Medicine Fellowship. Dr. Chudik is an author of numerous peer-reviewed scientific articles and an expert reviewer for premier journals in the areas of orthopaedic sports medicine, arthroscopic surgery, and shoulder and elbow surgery. He is a Fellow of the American Academy of Orthopaedic Surgeons, member of the American Shoulder and Elbow Surgeons, American Orthopaedic Society for Sports Medicine and the Arthroscopy Association of North America.
As an active inventor, Dr. Chudik holds seven patents and develops minimally-invasive arthroscopic surgical procedures, instruments and implants to provide better outcomes for his patients. His efforts continue to influence and advance the field of orthopaedic surgery and arthroscopy. He founded the Orthopedic Surgery and Sports Medicine Teaching and Research Foundation, a nonprofit 501 (C)(3) organization to help keep people active and healthy by funding unbiased research, education, conferences and residency programs.
Dr. Chudik also serves as a team physician for several Chicago-area athletic programs and clubs, and is a member of the High Performance network for USA Swimming. In his free time, he enjoys traveling, biking and spending time with his wife and four children.

Larry Frank Jr., PT DPT CSCS
ReLive Physical Therapy
Larry Frank Jr., PT, DPR, CSCS, Clinical Director, ReLive Physical Therapy in Glenview, IL. Larry is driven by his passion for improving the quality of healthcare delivery and empowering patients through movement and education. Larry provides his patients with the knowledge and tools they need to successfully manage their conditions. He has spent the last 5 years gaining leadership experience, as a physical therapist at NorthShore University HealthSystem and Northwest Community Healthcare. During that time, his focus was in outpatient orthopedics treating a variety of musculoskeletal injuries with a focus on sports medicine.
Larry has proven himself as a leader in this field. He has used his skills and knowledge to provide clinical instruction to multiple physical therapy and physical therapy assistant students. At Northwest Community he led communication with Harper College Athletics while treating collegiate athletes, as well as with the Harper College recreation center to drive two-way participation and membership.
At NorthShore, Larry participated in multiple special interest groups to level-up his knowledge and expertise in multiple areas including athlete lower extremity injury prevention, weight lifting, overhead athletes, and pediatric orthopedics. He has been a major collaborator in multiple department-wide projects including: improving communication between physical therapy and sports medicine surgical departments, developing and implementing return to sport protocols for ACL reconstruction, and creating educational content for other clinicians. Additionally, Larry was a leader in the implementation of Blood Flow Restriction Training to multiple clinical sites.
To add to his credentials, Larry is a Certified Strength and Conditioning Specialist (CSCS) and is a member of the National Strength and Conditioning Association (NCSA). He received his Doctor of Physical Therapy in 2017 from Northwestern University Department of Physical Therapy and Human Movement Sciences and received dual Bachelor of Science Degrees in Physiology and Psychology with a minor in Economics in 2014 from the University of Arizona. Larry is available immediately and is now accepting patients. The company is in all major healthcare insurance companies and Medicare.

Danielle Sebek, DPT
ReLive Physical Therapy
Danielle graduated from Carroll University with a Bachelor of Science in Exercise Science. She continued right into Carroll University's physical therapy program to earn a doctorate in physical therapy. She enjoys treating each patient with an individualized program utilizing functional and therapeutic exercises in combination with manual techniques to return patients back to the highest level of function. Because of her clinical success, she has been promoted to clinic director of ReLive Physical Therapy's Joliet and new Bloomingdale office. She has taken continuing education courses to gain further knowledge in treating various orthopedic and vestibular conditions and is also Dry Needling Certified. Her clinical experience includes treating patients for post-op care, musculoskeletal pain, and vestibular and balance conditions. In her free time, Danielle enjoys powerlifting and coaching gymnastics.

Mark Cohen, MD

Originally from Chicago where he graduated valedictorian from Niles West High School, Dr. Cohen received his bachelor's degree from Stanford University, graduating with highest distinction and departmental honors. He then attended Harvard Medical School, where he graduated Magna Cum Laude with honors in a special field. After serving an orthopaedic residency at the University of California, San Diego, he completed a hand, upper extremity and microvascular fellowship at the Indiana Hand Center, one of the largest and prestigious hand centers in the country.
Dr. Cohen is the author of more than 146 medical articles, 58 book chapters and he has written a textbook on Hand and Wrist injuries. In addition, he is an editor of Green's Operative Hand Surgery, the most widely read and recognized Hand and Elbow Surgery textbook in the world. He has participated in over 325 continuing education courses where he has taught other doctors, serving as Course Chairman 50 times. For the last 28 years, he has been the Director of the Orthopaedic Hand and Elbow Section at Rush University, and he currently serves as the Director of Orthopaedic Education in the department. He has won several research, teaching and achievement awards and is a member of 16 national and international medical societies and associations.
Dr. Cohen was again selected one of Chicago's "Top Doctors" in Hand and Orthopaedic Surgery by Chicago Magazine in 2022. He was previously honored in this fashion in 1997, 2001, 2004, 2006, 2008, 2010, 2012, 2014, 2016 and 2018 and 2020, every year the "Top Doctors" edition has been published. U.S.News & World Report rates him in the top 1% of physicians in his specialty in the nation. Dr. Cohen was recently named the 2017 recipient of the Excellence in Clinical Service award by Rush University, which recognizes faculty who demonstrate continuous excellence in clinical care, leadership and scholarship.
His clinical and research interests focus on fractures and reconstruction of the hand, wrist and elbow with a special emphasis on minimally invasive surgery. Dr. Cohen has special training and expertise in complex elbow conditions including the throwing athlete (Tommy John surgery) and other sports related injuries. Dr. Cohen is a team physician and consultant for the Chicago White Sox and the Chicago Bulls. In his free time, he enjoys tennis, cycling and in-line skating.

Jesse Butler, MD

Jesse P. Butler, MD, CIME, ABOS certified, fellowship trained practicing spine surgeon. Dr. Butler treats adult patients with spinal conditions, including complex revision spine surgery for failed back surgery syndrome. With more than two decades of successful spine surgery experience, Dr. Butler has the training, technical skill and diagnostic expertise to provide the best possible results for his patients. He applies an evidence-based, multidisciplinary approach to manage spine disorders. Dr. Butler graduated from Southern Methodist University and received his Doctor of Medicine from UHS/The Chicago Medical School. He completed his Orthopaedic Surgery residency at McGaw Medical Center at Northwestern University. He performed his spine surgery fellowship at Rush-Presbyterian – St. Luke’s Medical Center. He has performed over 5,000 spinal procedures on patients from around the world with a special emphasis on patients in Chicago, Illinois and areas surrounding. Dr. Butler is a past member of the Illinois Workers’ Compensation Commission Medical Fee Advisory Board (2004-2011).

Catherine Krenz Doan (Moderator)

Catherine Krenz Doan is a partner with Rubin Law Group, Ltd. She concentrates her practice in the area of workers’ compensation law. Ms. Doan graduated from the University of Wisconsin - La Crosse with highest honors and received her law degree from IIT-Chicago Kent College of Law magna cum laude. She is a member of the Order of the Coif. While in law school, Catherine was a notes and comments editor for the Chicago Kent Law Review.She has authored various articles for the IICLE, the Illinois Trial Lawyers Association, and the Illinois State Bar Association. She has also participated in seminars for IICLE. Ms. Doan is a member of the Illinois State Bar Association, the Workers’ Compensation Lawyers Association, the Chicago Bar Association, and the Illinois Trial Lawyers Association.
Doan has been selected by her Illinois peers as an Emerging Lawyer from 2016 to present, a distinction awarded by Leading Lawyers.
